32 bit file handles

No, it's the stick itself that's wrong :-)

Basically, 8-bit file handles were a mistake; even Jonathan acknowledges
that. It seems wrong to compound that mistake to alter the grammar to work
round it. Therefore the xxx32.h are manually generated.

I've now done most of the work, NEEDSATEND works :-) 
I've not yet tried compiling against the new headers.
